Thank you for your interest in the Night at the Races.  You may sign-up on-line or mail this information to the Foundation at the address below.

         (a) your contact information,

         (b) for parties of four or more, you may optionally request reserved seating,

         (c) your payment via mail or on-line.

(B)  Optional Table Reservation Request:

You may optionally request reserved seating for a party of four (4) or more guests for the Night at the Races.   Please make your request, and provide for payment for each guest in your party, by no later than February 26, 2005.

Please note: we can not reserve your table until we receive payment from each guest in your party.

Please include any information which might be helpful for us to contact the primary contact and as much information as possible for the other guests in the group.  We will send you, and each additional e-mail address you list below, an e-mail with your table information once we have confirmed your request and received payment.
